Application

Fill out all sides of the application form and sign. Answer every question or make a mark indicating that you have read the question. All of our forms can be found here.

Special Instructions

Any special instructions such as not letting your child go outside on a particular day, someone other than an authorized person picking up your child, etc., should be noted to your child's teacher in writing. If you want someone else to pick up your child you must provide written permission to the director. Photo identification will be required.

Personal Articles & Clothing

• The Center should have a complete change of clothing in your child's bin at all times (please replace ASAP if used).
• Parents provide one standard cot sheet and a small blanket for their child's personal use (to allow each child a comfort item from home).
• During summer months we will have a sprinkler day. Please bring a swimsuit and towel for your child (apply sunscreen at home if needed).
• Dress your child in comfortable clothing and tennis shoes so he/she can play outside (weather permitting). No open-toed shoes. Do not allow your child to wear coats or other articles of clothing with strings.
• Parents should label everything they send to the center. We try very hard to keep our centers organized - this will assist us.

Toys

For safety and sanitary reasons we cannot allow children to bring toys, money, gadgets, etc., to the Center.

Payment

Payment must be made in advance the first day the child attends. If payment is not received on Monday there will be a $10.00 late fee and your child will not be allowed to attend until payment is made. Full payment is required regardless of the child's attendance. The regular rate is for 10 hours of child care each day. Each child will get one week of vacation each year after they have been enrolled one year (2 week written notice required). Vacation must be taken in full weeks. A two week notice is required upon withdrawing your child from the Center. If notice is not given, you will be required to pay for those two weeks. See rate sheet for additional information.

Health & Illness Policy

• We cannot admit a child who has any contagious symptoms: fever, vomiting, diarrhea, open sores, unexplained rashes, infectious drainage, etc.
• If you child becomes ill at the Center, we will notify you and the child must be picked up.
• Your child must be symptom free for 24 hours before he/she can return to the Center.
• Parents must sign the Medicine Permission Chart for us to administer medicine.
• Medication is given at 11:00am and 3:00pm
• An Immunization Certificate is required upon enrolling and must be kept up-to-date.
• Parent or Guardian will be notified of any significant occurences or problems which affect your child, including exposure to communicable disease.

Nap Time

Nap time is between 12:30 and 2:30pm daily. (Time varies according to age and location).

Mornings

Please have your child at the Center by 9:30am. If your child is going to be absent, please notify the Center before arrival time.

Food

Outside food is not allowed in the Center pursuant to State law. If you bring something for your child's birthday, for the class, it must be non-edible.

Infants

Parents are to provide disposable diapers, wipes, changes of clothing, baby food, and prepared bottles marked with the child's name. Parents may choose to use formula supplied by the center. Whole milk will be provided after the child's first birthday. Infant cereals are provided.

Dismissal

Southside Christian reserves the right to dismiss a child of any reason. The use of tobacco, profanity, violence, alcohol, illegal substances, weapons or firearms in or around the facility by any adult or child will result in dismissal from the center.
